Did you happen to record any episodes previously where the guide data had a generic description and you edited the metadata on them?
No I do not bother with Edit Metadata yet ... All I do is record to an SSD then move to secondary storage. The only features I use is Library Collections and Favorites. Of course, Client Settings.
I am thinking maybe is that POP shows the same episode back to back ? Maybe it screws with DVR logic.
Back-back or overlapping doesn't matter. It uses the ProgramID of the episode to prevent duplicate recordings. If the same episode was aired multiple times, it would only record the first one.
Was just thinking you may have recorded multiple airings where the guide data had no Season number, Episode number or Episode title. The description would be the generic show description.
They would appear in /dvr/programs with the SHow ProgramID SH029715140000 instead of the EPisode ProgramID like EP029715140001
The DVR will not record multiples of an EPisode ProgramID like EP029715140001, but will record multiples of a SHow ProgramID like SH029715140000.
Looks like I will be getting duplicates for all recorded episodes ... better to have 2 than none ... I will just chalk it up as being investigated.

And no matter how hard we tried, we could never get Channels DVR to do that.
The ones that are marked as watched not duplicated are scheduled. Something about POP and Channels DVR first TVE now duplicates ... 
Can you search /dvr/programs output for those upcoming program ids to see if the DVR thinks they were already recorded before or not

I knew I had this happen to me before, just had to search a bit to find the posts.
Not sure if it's related to your issue.
My situation is totally different these recordings were recorded during the summer and are still in the library, and they are re-recording in the fall/winter
