Provider - DirecTV

DirecTV has recently changed their backend authentication server/login process, and it has been causing some problems. Updating to the latest pre-release, removing the source, and re-adding DirecTV seems to be working for most people as a fix. DirecTV also allows for creating "secondary" accounts in addition to the primary account. For most networks, TV Everywhere access is granted to these secondary accounts.
