I'm not sure when this changed, but I've noticed it on multiple Macs now, in Safari. These icons used to clearly indicate "PIP" and "AirPlay" and now they're just squares:

Did you get a solution? I have the same issue. I'm on version 2022.11.15.1908.
Unfortunately not. A few days ago I updated to macOS Ventura 13.0.1, and I'm still seeing those blank squares in Safari where "PIP" and "AirPlay" icons used to be.
Just got a new Mac and didn't migrate any data over, just logged in fresh as a new user, browsing via Safari, and still see the same squares. So now I think the DVR code is missing icons for everybody?
