I’ve only seen this happen once - but I wanted to mention it here.
Last night, about 9:50pm, I started watching a recording that was made earlier that evening. I remember pausing it once for maybe 10 minutes, otherwise watched it pretty much straight through.
At 10:15pm, while I was watching this, the DVR upgraded itself to a newer version v2017.01.28.0312. I only know this from looking in the logs - watching my show was unaffected.
After I was done watching the show on tvOS (around 10:30 or so), I clicked Delete, and the show was removed from the recordings list on the TV. This is when I noticed the first odd thing - clicking Menu didn’t show the DVR available at this point. I then clicked around (might have started watching something live?) and the DVR menus came back immediately.
Later, I looked at the DVR recordings on the web site and noticed the show I had deleted on the TV was actually still there. I deleted it from the DVR site and it went away as expected.
There isn’t really anything interesting in the log - just the usual messages you see when the DVR updates:

So anyway, I thought I would mention all this - when the DVR updated while I was using the TV app, it seems things got a little wonky there for a bit.
