Have lip-sync as well. Didn't occur when I first installed the ATV but came up a number of months ago. I don't have it when using any other clients/devices that point to Channels DVR on my Synology. Source of the stream is HDHomeRun which is on the same switch as Synology/ATV/HDHR. Using iPad or Mac there's no delay. With ATV it's about 1/10th of a second. I plan to enable the Channels client on my Zidoo player to see if it shows up, as well as the HDHomeRun app. If these work I'll know the blame is with the ATV.
You can try changing Settings > Playback > Advanced > Audio Driver in the app
