Are there particular settings I need to disable/enable on chrome browser?
I installed in another computer and it is working. We can close this issue.
What version of Chrome on each computer?
I subscribe to SNY and can confirm that I can watch NY Mets games live using the NBC Sports App. But when I enter my Optimum login using TV anywhere, it says "no login form found" under SNY channel 6162 and doesn't seem to find a NBC Sports app to attempt to login. Please help as this is the only channel missing from my dvr!

I have a hunch there might be a mismatch with the "optimum ID" I'm using, even though there only appears to be one on the account.
Try it on nbc.com/live and figure out which combination works. Maybe you need to pick Altice TV from the list, which I think is now Optimum TV?
Thanks, it was indeed Altice One. Seems to have found most channels after I used that in conjunction with new ID.
Seeing this on a few channels however:
Happens sometime when adding a new provider.
Those all happen to be Discovery Network channels.
Individually rescan those channels one at a time using the gear dropdown to the right of them.
