I am having trouble clearing these out. Does anyone have this issue?

Have you tried updating to the latest pre release?
If that doesn't fix it, I would submit diagnostics from the server.

yes, I get this issue often when I play around with my sources / m3u's and passes. I've concluded that the recording failure happened with a previous iteration of one of those things and no longer indexes the same.
I have cleared them by pausing the pass associated with the recording.
usually once the pass finishes pausing it's schedule, the failed recording entries clear themselves, and you can resume the pass.
