I've been updating my guide as theres lots of last minute changes recently to the guide.
Strangely the top shelf on the Apple TV showed the programme I was expecting. Inside the app I saw different guide data all together.
I had to restart the app (double tap home and swipe up) and it all looked good. Difficult to keep testing as it relies on actual guide changes!

The app will not fetch guide data if it has gotten a recent fetch. This is by design to keep things speedy. So if the app hasn't been quit, then it will keep it's pattern on when to fetch guide data.
The TopShelf extension is basically its own app. Whenever you focus the Channels icon, tvOS basically restarts the 'app' from scratch, so it will always pull guide data fresh.
This explains why you're seeing this.
Since you're manually refreshing the guide data so much, forcing the app to quit to force a guide update is the proper steps you should be taking.
