Twice now Channels DVR has recorded the wrong show in a one week period. This is on an ubuntu server and Amazon Fire front end. Is the guide not updating correctly? This does not happen in Plex or other software. Is this a known issue? Is there a fix?

What show are you referring to? What is the source? It’s most likely that the guide data is incorrect (or the network changed what they were airing at the last minute), but without more details it’s impossible to tell anything.
It was an NBC show called La Brea. The guide indicated that it was shown on NBC last Saturday at 8pm. However, looking at TV guide says that The Wall is what is scheduled for that time slot. So it appears that the guide data was incorrect. But why?
Just thinking out loud: maybe your guide is mapped to the wrong NBC channel?
What are the call letters of your local NBC?
In my experience, sometimes local affiliates move things around on their schedule to work around stuff like sporting events, political events, etc. in their local markets but don't appear to do anything to update any of the EPGs. I missed nearly a whole week of one of my favorite daily shows because my local station decided to air political rallies instead, and they bumped said show to one of their crappy low-res sub-channels. I only figured it out towards the end of the week because I happened to watch one of their news broadcast and they mentioned the change. So I had to set my DVR to record Hogan's Heroes or somesuch in order to catch it.

As far as I recall, there was no sporting event that went long. Also, if guide data was mapped incorrectly, then other shows wouldn't record correctly, but they do. It's just been a couple of shows so far that were incorrectly recorded and they aren't even on the same channel.
Channel DVR uses Gracenote guide data.
You can see the same on zap2it.com who gets their guide data from Gracenote.
If you notice the problem with another show, you can look back up to a day at zap2it.com to see what the guide data says was airing.
It happens, guide data isn't 100% accurate.
I had it happen again and this time I'm pretty sure that it's a channels DVR issue. Here's what happened.
I watched The Addams Family on MeTV this afternoon. Worked fine. While watching the Addams Family, I scheduled Vega$ to record later in the day via the guide.
Tonight, I see the recording listed as if it recorded correctly, but it's something recorded from a spanish language channel and all I got was audio with a black screen. Super weird. I love this software's interface and performance. Currently on the free trial and I want to give them my money, but seriously what is going on? I've been using homebrew DVRs since the early days of MythTV and I've never experienced such recording errors.
Please supply the relevant logs, otherwise everything is anecdotal and can't be addressed.
Sounds like the guide data is wrong?
What does the DVR log show for what actual channel was recorded?
It appears that I have 3 different MeTV channels listed in my guide for some reason. 35.1, 35.6 and 43.1. It doesn't seem likely that there really are 3 of those channes? In any event, the Addams family I watched was on 35.1 and vegas was on 43.1. The log shows no data for the 43.1. What should I do? Setup the guide data and scan again?

Looks like you're not getting a good signal for 43.1
You may want to simply disable some of these non working channels via the lineup on your HDHR
For the ones that work but have wrong/missing guide, you can click the gear icon next to your HDHR and select Manage Lineup, then set the correct guide data for each station. You can also hide channels in that same interface.
I guess eventually it did succeed in tuning that channel, but even then the signal strength is under 60% so it may not be watchable.
One thing that seems strange is that when I go to 'manage lineup' from the HD Homerun tuner I see that 43.1 is actually a Univision next to it but has the MeTV icon. Watching it live I see Univision. So the guide data or channel mapping is wrong.

Yes, I did that, thank you. In the web interface, I'm trying to hide certain channels I never watch. This does not seem to hide them from the app on the Amazon Fire TV though. Is that normal? Do I need to hide it on each individual device?
Channels lets you choose whether or not changes like this at the server/client end get synchronized to the client/server. I can't remember exactly where to find this option. I'd start with the server config page (ie. the port 8089 page you bring up in a browser).
I think this is a setting on the client side. If you go into the app settings and look around in the courses area you should be able to find an option to sync channel state to or from the server.
