All scheduled recordings have recently started to fail.

You will need to post more information other than "Recordings are failing".
What is the source of the recordings, OTA, TVE, Custom channel?
What does the server logs say?
Have you submitted diagnostics from the server?
Have you run the Troubleshooting page on the server, is there any red marks or issues shown?
Have your followed the troubleshooting steps outlined in the Channels DVR documentation?

Thanks! Your recommendations have seemed to have lead me to the issue. Many TVE HULU channels are gone upon a rescan. Is this a change in the HULU package?
Try these steps for TVE issues
What stations? I would try removing HULU and readding it back. Or just do a channel scan and see what happens.
