My TV Everywhere was previously working, but had stopped working so I deleted the source and tried to re-add. Am using correct credentials but getting this error screen:

Yes, most of them. I can login directly to live stream from a provider using FIOS creds. This error does not look related to credentials though.
Did you do all of the steps (including sending logs to the devs)?
Yes, I sent logs. Weirdly, its now doing what you see in the log below and actually crashes the Channels server!
